• Name Change: For legal name changes, a Name Change Form should be completed. Once this is completed, HR Direct, I-9 and GIC Benefits, if applicable, will be updated. UMass Lowell requires that you submit a copy of the documentation that legally validates your change of name. Documents that can be submitted are Social Security card, drivers license or passport. Submit the Name Change Form and documentation by visiting AskHR.
  • Race/Ethnicity: Employees are encouraged to self-identify race and ethnicity for workforce diversity planning, by contacting Human Resources by visiting AskHR. This information is considered in aggregate and not in connection with your name. 

HR Direct Self-Service may be used to designate any of the following. Log into HR Direct. Under the tab on the left, Employee Self Service, click the Personal Details tile.
